Ozempic, Wegovy and related medications — what happens to the body after rapid weight loss?
GLP-1 receptor agonists — including semaglutide (Ozempic, Wegovy) and liraglutide — have become widely used for significant and relatively rapid weight loss. Losses of tens of kilograms over a comparatively short period produce notable changes in body shape: skin laxity, changes in breast volume and contour, and abdominal changes broadly similar to those seen after bariatric surgery.
How does GLP-1-induced weight loss differ from bariatric surgery?
Despite similarities in physical outcome, there are important clinical differences between the two patient populations. Published research from our group — including a comparative case series in Plastic and Reconstructive Surgery — examines the differences in surgical outcomes between patients who lost weight with GLP-1 agonists and those who underwent bariatric surgery.
Factors such as the rate of weight loss, weight stability after discontinuation of the medication, the effect of the drug on tissue healing, and the quality of the remaining skin envelope are all relevant to surgical planning and expected outcomes.
Which procedures are most common after GLP-1-induced weight loss?
Abdominoplasty: Removal of redundant skin and adipose tissue from the abdominal region, with restoration of abdominal wall muscle tone. Following significant weight loss, this procedure may involve management of substantial skin excess and requires precise planning of resection margins.
Breast reduction and mastopexy: Rapid weight loss typically produces marked reduction in breast volume and ptosis. Breast reduction or mastopexy restores proportion and support.
Combined procedures: In selected cases it is possible to combine procedures in a single surgical session — subject to careful assessment of safety considerations and complication risk.
When is the right time to consider surgery?
A central principle in planning body contouring after weight loss is weight stability. It is recommended to wait until weight has been stable for at least three months before surgery. Continued weight loss during the surgical period may alter the outcome and result in additional skin redundancy.
Regarding discontinuation of GLP-1 medication before surgery: professional recommendations are evolving. There is interest in discontinuing the medication around the time of surgery given its potential effect on gastric emptying and tissue healing. This will be discussed in detail at the preoperative consultation based on your individual medical profile.
What about resuming the medication after surgery?
Body contouring surgery is not a substitute for the medication and does not guarantee weight maintenance after discontinuation. Patients considering body contouring are welcome to raise this question at consultation — to obtain a complete and realistic picture of what to expect.
